No One Should Die in a Confined Space: OSHA Training That Prevents Fatalities

Every year, workers enter tanks, manholes, silos, vaults, and other enclosed areas for routine maintenance, cleaning, or inspection. What should be a straightforward task too often ends in tragedy. These incidents are not accidents—they are preventable. According to OSHA, proper identification, permitting, atmospheric testing, ventilation, and training eliminate the risks. Yet failures in these basic steps continue to claim lives.


What OSHA Defines as a Confined Space and Permit-Required Confined Space (PRCS)


OSHA’s standard (29 CFR 1910.146 for general industry and 1926 Subpart AA for construction) clearly defines a confined space as any space that:

  • Is large enough and configured for an employee to bodily enter and perform assigned work;

  • Has limited or restricted means for entry or exit (such as tanks, vessels, silos, storage bins, hoppers, vaults, or pits); and

  • Is not designed for continuous employee occupancy.

A permit-required confined space (PRCS) is a confined space that has one or more of these additional characteristics:

  • Contains or has the potential to contain a hazardous atmosphere;

  • Contains a material that has the potential for engulfing an entrant;

  • Has an internal configuration such that an entrant could be trapped or asphyxiated by inwardly converging walls or a floor that slopes downward and tapers to a smaller cross-section; or

  • Contains any other recognized serious safety or health hazard.


PRCS entry requires a written permit, atmospheric testing, hazard controls, an attendant, and rescue procedures. Non-permit spaces can become permit-required if conditions change—yet many employers skip this critical evaluation.


The Human Cost: Hundreds of Preventable Deaths in the Past Decade


Confined-space fatalities remain stubbornly high. The Bureau of Labor Statistics (BLS) recorded 1,030 deaths involving confined spaces from 2011 to 2018—an average of roughly 100 per year. More recent years continue to show dozens of fatalities annually, with the same preventable patterns repeating.


Tragically, more than 60% of these deaths involve would-be rescuers who rush in without proper equipment or training—turning a single incident into a multiple-fatality event.


2025 made headlines with preventable losses:

  • In January, 35-year-old Dalion Ambler died inside a railroad tanker car in Twinsburg, Ohio, after entering an oxygen-deficient atmosphere (as low as 5% O₂). The contractor failed to follow permit-required confined space procedures, including testing and ventilation.

  • In August, two workers in Mobile, Alabama, were overcome by sewage gases while installing sewer lines in a manhole. A third worker self-rescued; the employer had no confined space entry program, training, or emergency plan.

  • Days later in Trinity County, Texas, three workers died in a sewer lift station manhole from toxic hydrogen sulfide and sewage gas. Two entered to rescue the first.

  • Also in August, six workers died in a manure pit on a Colorado dairy farm—another confined-space tragedy involving rescuers.


These incidents share the same root causes: no permits, no testing, no training, and no rescue plan.


OSHA Training Requirements vs. Company Failures


OSHA mandates that employers train every employee involved in confined space operations—entrants, attendants, entry supervisors, and rescuers—so they gain the “understanding, knowledge, and skills necessary for the safe performance of their duties.” Training must occur:

  • Before the employee is first assigned duties;

  • Before there is a change in assigned duties;

  • Whenever there is a change in permit-space operations that presents a new hazard; and

  • Whenever the employer has reason to believe that employees are deviating from procedures or that their knowledge is inadequate.


The program must cover hazard recognition, atmospheric testing and monitoring, ventilation, PPE, permitting procedures, communication, and rescue/emergency services. Employers must also practice rescue operations annually.


Yet too many companies treat compliance as optional. They skip permits to “save time,” fail to test atmospheres, provide no hands-on training, or rely on untrained coworkers for rescue. OSHA citations in 2025 repeatedly cited these exact failures—lack of programs, training, ventilation, and retrieval systems—leading to proposed fines in the hundreds of thousands of dollars.


Why Computer-Based Training (CBT) Alone Is Not Adequate for PRCS


Many employers turn to computer-based or online training programs thinking they provide a quick, low-cost way to “check the box” for OSHA compliance. However, OSHA has repeatedly stated that computer-based training by itself does not meet the intent of its training requirements, particularly for permit-required confined spaces.


OSHA emphasizes that effective training must result in employees mastering the necessary skills through hands-on practice, real-time interaction with a qualified trainer (to ask questions and receive immediate feedback), and opportunities to demonstrate competency with actual equipment in a safe environment. Sole reliance on CBT lacks these critical elements: it cannot replicate the muscle memory of donning PPE, operating gas monitors in realistic scenarios, practicing rescue techniques, or evaluating site-specific hazards.


In confined space work—where seconds count and conditions can change rapidly—workers need more than passive video lectures or multiple-choice quizzes. Without hands-on components, employees may understand the theory but fail to perform safely when it matters most, contributing directly to the preventable fatalities we continue to see.
